  Service Application and Agreement
Form used to apply for new membership in Wylie Northeast Special Utility District. This form will not be accepted until all fees have been received at the WNSUD office.
 
  Standard Easement & Right-Of-Way
Easement and right of way including temporary construction easement.
 
  Non-Standard Service Application
Form used to request water service for a new development.
 
  Temporary Service Request & Agreement
Builders request for temporary water service.
 
  Deferred Payment Agreement
Agreement to extend time for payment of outstanding water bills. Must be agreed to by the Manager or Board of Directors.
 
  Installment Agreement
Agreement by member to make installment payments to retire an outstanding debt to WNSUD. Must be approved by the Board of Directors.
 
  Meter Test Authorization & Test Results
Request by a member to have their water meter tested for accuracy and agreement to pay for testing should the meter prove to be accurate within state standards.
 
  Service Inspection Certificate
Form to be completed and attested to by a state certified inspector before permanent water service may be connected.
 
  Emergency Leak Agreement
Form to be signed by member authorizing WNSUD to shut off water if and when a leak on the member's side of the meter is detected to protect the member from excessive water use and high water bills.
 
  Miscellaneous Rate Order and Transaction Forms
In addition to the above forms, includes the following forms: notice to owner of rental property, notice of water rationing, confidential utility records form, notice of requirement to comply with subdivision policy, notice of returned check, request for service discontinuance, easement denial form and affidavit, termination notice, notice of insufficient information, notice of rationing violation, second notice of rationing violation and penalty, notice of termination for continued rationing violations, hydrant meter service request and landlord billing agreement.
